Erie Family Health Center is a nonprofit, federally qualified community health center that provides comprehensive primary healthcare services to the economically disadvantaged and medically underserved. To assist our efforts towards competent and compassionate care, we seek a LPN to provide care within the Illinois Breast and Cervical Cancer Program.
The selected LPN will provide clinical oversight for patients needing services with the Illinois Breast and Cervical Cancer Program (IBCCP). Responsibilities include reviewing all womens cases to assure appropriate follow-up; provide case management to all women with abnormal test results; reviews and enters all abnormal test result data into Cornerstone; facilitates communication and coordinates services with members of the health care team; attends mandatory meetings and collaborates with EFHC and DHS representatives; and serves as a liaison between the Director of Health Center Operations, Nurses and Providers.

| Job Requirements |
| Qualifications include current LPN license, minimum one year experience with outpatient nursing, and Basic Life Support certification. Since the community we serve is culturally diverse, you must demonstrate cultural competency. Bilingual Spanish/English skills are preferred. Must also have excellent multi-task, people, communication, and typing skills. |
